Surgical Safety and the WHO Checklist, start to finish.
This training ensures you understand and properly use the World Health Organisation Surgical Safety Checklist and the Five Steps to Safer Surgery in our theatre. The checklist prevents serious surgical harm including wrong site surgery, wrong implants and retained instruments. This is the same module CareStream subscribers get, written for care settings rather than adapted from a generic course. It runs about 35 minutes and ends with a 20 question assessment.
- The WHO Checklist and Why It Exists
The WHO Surgical Safety Checklist is a proven tool that reduces surgical deaths and complications by catching errors before they reach the patient.
- The Five Steps to Safer Surgery
In the UK we use the Five Steps to Safer Surgery framework, which adds a team briefing at the start of the operating list and a debrief at the end.
- Sign In Before Anaesthesia
The Sign In happens before anaesthesia is given.
- Time Out Before Incision
The Time Out is a deliberate pause immediately before the skin incision when the entire team stops and gives full attention.
- Sign Out Before Leaving Theatre
The Sign Out happens before the patient leaves the operating theatre.
- Speaking Up and Flattening Hierarchy
The checklist only works when every team member feels able to speak up if something is not right, regardless of their role or seniority.

What your team will be able to do.
- Describe the three phases of the WHO Surgical Safety Checklist and what is checked at each phase
- Explain how the Five Steps to Safer Surgery framework applies across an operating list in this hospital
- Identify situations where you must speak up to stop or pause a procedure because of a safety concern
- Apply the correct checks for patient identity, site marking, consent, swab counts and specimen labelling in realistic theatre scenarios
- Recognise the difference between meaningful checklist use and tick box compliance

What staff come away with
The WHO Surgical Safety Checklist has three phases: Sign In before anaesthesia, Time Out before incision and Sign Out before leaving theatre, plus team briefing and debrief in the Five Steps model
The checklist prevents never events including wrong site surgery, wrong implants and retained instruments, which cause catastrophic harm and must be reported
The Time Out is a genuine team pause where everyone stops, introduces themselves and confirms patient, procedure and site together with full attention, not a rushed tick box
Every team member must speak up if they notice a problem, and the hospital supports you in challenging seniors when safety is at risk
Swab, needle and instrument counts must be correct before the patient leaves theatre; a discrepancy must stop the Sign Out until resolved
The checklist only works when the team genuinely engages with each check, looks at the patient and notes, and communicates for real
Three steps, and nothing to set up.

How long does Surgical Safety and the WHO Checklist training take?
About 35 minutes, across 6 short lessons, followed by a 20 question assessment with a pass mark of 80%. Staff can stop and pick up where they left off.
What does the Surgical Safety and the WHO Checklist module cover?
6 lessons: The WHO Checklist and Why It Exists, The Five Steps to Safer Surgery, Sign In Before Anaesthesia, Time Out Before Incision, Sign Out Before Leaving Theatre and Speaking Up and Flattening Hierarchy. Each one pairs the teaching with a care scenario and a quick check.
How often do staff need to refresh Surgical Safety and the WHO Checklist?
CareStream sets this module so it renews every year. Staff and managers are reminded before a certificate lapses, so the refresher is booked before the gap appears.
Is Surgical Safety and the WHO Checklist completed fully online?
Yes. It is knowledge based, so staff complete the lessons and the assessment online and receive their certificate at the end. There is no practical sign-off.
Do I need a CareStream subscription to buy this?
No. This page exists precisely so you do not. You buy the licences you need, your staff complete the module in the CareStream hub, and that is the end of it. There is no trial to start, no card kept on file, and no subscription that begins quietly afterwards.
If you later decide you want the full platform, anything your staff have already completed carries across into your account.
What does one licence actually cover?
One licence covers one named member of staff for this one module, including the lesson, the assessment, any follow-up questions triggered by a wrong answer, and their certificate. Licences are not shared between people, because the completion record has to belong to an individual to be worth anything at inspection.
How quickly can staff start?
Immediately. The licences are on your account as soon as the payment clears, and staff get their invite the moment you add them. There is no onboarding call and no setup period.
Can staff complete it in another language?
Yes. Each staff member has a first language on their profile, and the lesson, the questions and the feedback all arrive in it. They answer in that language too. What you see as a manager stays in English, so the record reads the same whoever completed it.
What if someone fails the assessment?
Nothing is lost. A wrong answer triggers a short follow-up lesson on that specific point, and they retry it. The record shows the improvement from the first attempt to the latest, which is stronger evidence than a single pass mark, because it shows the gap was found and closed.
Can I buy for more than one service?
Yes. Buy the total number of licences you need and allocate them across your services when you add staff. If you are licensing for a group and want an invoice rather than a card payment, get in touch and we will raise one.
Licence any of these the same way.
Same price, same terms, no subscription needed.
