Medical and Health Factors in Fall Risk
Our fall risk assessment includes reviewing medical conditions that increase fall risk. Conditions like osteoporosis make bones fragile, blood pressure problems can cause dizziness, and sensory issues affect balance. All residents have regular medication reviews and health checks. If we identify problems with eyesight, hearing, or balance, we refer the person urgently to the appropriate specialist or their GP. Managing these health factors is essential to preventing falls.
Mobility Assessment and Support
We conduct a full assessment of each person's mobility, including getting in and out of bed, bath, and chairs. We check if they need mobility aids and if they can use them properly. The assessment also reviews their footwear and ability to dress themselves. People with mobility problems must have up to date aids and know how to use them correctly. Some residents need staff to accompany them when moving around to prevent falls.
Individual Care Plans for Fall Prevention
Each resident has an individualised care plan based on their fall risk assessment. This plan includes specific interventions to reduce or eliminate their particular risks. Examples include ensuring people do not stand up from chairs too quickly, providing help getting out of the bath where needed, and making sure those with mobility problems have the right aids. The care plan is personal to each resident and you must follow it exactly.
Environmental Hazards and Safety
Our fall risk assessment includes checking the environment for hazards. We may use an occupational therapist to identify risks and recommend how to address them. Common hazards include wet bathroom floors, poor lighting, clutter, loose rugs, and obstacles in walkways. We must keep areas clear and well lit.
